Appointments

At your first appointment, your therapist will do an extensive (approximately one hour) evaluation of your injury or condition. During this evaluation, the treating provider will devise the best care plan for you that will include: duration of care, goals to be met, hands on care to be provided, and a home exercise program.

What you’ll need for your first appointment:

• Wear comfortable clothes and closed toe shoes
• Bring your insurance card(s)
• Bring your drivers license or photo ID
• Bring a list of current medications
• Bring your doctors order for physical therapy services (unless entering care under Direct Access)
